Visit this resource designed and built by AMC volunteers specifically for the use of chapter leaders. Here you will find latest versions of our volunteer release agreements and waivers, accident report, crisis communication chart for chapter volunteer activities, Leadership Guidelines and Requirements, LTI information, as well as the recently approved Chapter Youth Program Guidelines and Sponsorship Agreement.

In addition, a volunteer form for reporting trips in the WMNF, Release Agreement FAQ's, and other relevant documents are in the works.

Forms are in either the PDF or Word formats, along with a link for the Adobe Acrobat Reader so you can read and download the documents for use within your committees and volunteer communities.

Forms

AMC waiver form the official 'sign-in' form used for all activities
Expense form form for reimbursement of leader expenses such as phone calls for trips, handouts, food for planning meetings.
Scholarship application form form to apply for partial or full reimbursement for WFA, CPR, and advanced training.
Trip financial form form submitted to committee chair for trips that charge participant fees.
Hiking trip listing form form used submit the description of a hiking trip to the hiking committee chair.
Hiking trip screening form guide to screening participants
Hiking trip report form form submitted after a trip containing details of the hiking trip, e.g. weather, trail conditions, problems.
SOAP note wilderness accident report form
Accident report form chapter accident form to be submitted to the chapter chair and AMC Outdoor Leadership Coordinator

Activity Participation Policy

"AMC encourages involvement in its mission and activities, through its membership, programs, policies and procedures. Our goal is to be a community that is comfortable, inviting and accessible for people of any age, gender, race, religion, ethnicity, ability, sexual orientation, or socioeconomic status.

Some AMC programs are designed for a certain age range of members or for a special activity or topic. However, any person who meets the minimum qualifications (skills, experience, fitness) established by the trip leader(s) for an activity is eligible to attend, if space is available."
